About The Position

The Patient Placement RN will collaborate with hospital staff and physicians to support optimal patient flow across the system. This position will be responsible for daily shift operations for all internal and external hospital transfers including PACU, ED and procedural areas, in accordance with clinical standards and guidelines. This is an onsite position that will require you are able to work one weekend a month.

Requirements

  • Associate of Science in Nursing
  • 3 years recent clinical experience as an RN in a hospital setting

  • 5 years recent clinical experience as an RN in a hospital setting with charge nurse or operations management experience
  • 3 years recent patient placement experience

Responsibilities

  • Responsible for overall patient flow and internal patient placement process across M Health Fairview:
  • Prioritizes patient placement based upon patient need and hospital capacity.
  • Coordinates, prioritizes and expedites internal transfers, transfers between all M Health Fairview hospitals, and transfer admissions from PACU, ED, and other procedural areas.
  • Coordinates, prioritizes and expedites external transfers.
  • Identifies and evaluates bed availability in all patient care departments.
  • Applies knowledge of hospital and unit admission criteria and type of care provided in each department and service to appropriately place patients using critical thinking skills.
  • Maintains and communicates the global picture of system hospital capacity.
  • Engages in improvement efforts designed to help the system and the SOC function more efficiently and improve patient care processes and outcomes.
  • Responsible for developing and maintaining relationships and working collaboratively with Acute Care site leaders including, but not limited to Charge Nurse, Nurse Managers, Director of Nursing, etc. in all patient care areas:
  • Understands staffing levels on the patient care units based on communication with the staffing office, unit leadership and the Administrative Nursing Supervisor(s).
  • Routinely provides recommendations to Manager of Patient Placement to ensure the patient placement process is operating effectively.
  • Communicates frequently with Emergency Department, PACU and procedural areas to prevent backlog of admissions and implement procedures that will avoid diversions.
  • Serves as a liaison for clinical and operational issues surrounding timely transfers and discharges to optimize bed utilization.
  • Maintains positive working relationships with provider groups and nursing staff.
  • Helps medical staff problem-solve around barriers to efficient placement, such as considerations of alternative hospitals, need for telemetry, isolation and other patient and system factors.
  • Keeps providers and external referring providers up-to-date on status and triage of patients awaiting placement, as appropriate.
  • Demonstrates ability to advocate for and promote care or service, adjusting approaches to reflect developmental level and cultural differences of population served:
  • Partners with patient caregivers in placement decision-making.
  • Knowledge of physical, psychosocial and cognitive developmental needs of populations served.
  • Communicates in a respective manner.
  • Contributes to a safe, secure environment.
  • Modifies clinical placement based on populations served.
  • Provides education to leadership regarding patient placement decisions.
