About The Position

POSITION SUMMARY : The Patient Placement Coordinator facilitates admissions to BMC from all points of entry, including direct admissions, post-operative surgical admissions, transfers, and emergency department admissions across the health system. This role completes pre-registration workflows, care team and bed assignments, and collaborates with the Transfer Center Nurse, Flow MD, Bed Control Nurse, and other care team members to ensure appropriate patient placement and timely transfers. The Coordinator monitors the real-time BMC census in Epic, participates in daily bed huddles, and follows direction from Central Flow and Capacity Transfer Center leadership to prioritize patients as needed, including during Code Help. Additional responsibilities include collecting demographic and insurance information for transfers, completing administrative tasks, utilizing a call center telephone system to communicate with internal and external customers, maintaining patient confidentiality, supporting handoffs to reduce errors, and mentoring and training new staff. Requirements

  • High school diploma or GED required
  • 2 to 4 years of prior healthcare experience required; or equivalent combination of education and experience.

Responsibilities

  • Facilitates admissions to BMC for patients from all modes of entry into BMC including direct admissions/ bed reservations, post-operative surgical admissions, BMC transfers, and emergency department admissions across the health system
  • Completes the pre-registration workflows for all direct admissions and transfers.
  • Completes care team and bed assignments based on level of care, patient placement and care team regionalization guidelines.
  • Collaborates with the Transfer Center Nurse, Flow MD and care team resident as needed to ensure safe and appropriate distribution of patients
  • Collaborates with the Bed Control Nurse to arrange timely transfer of patients to and from intensive care units as well as intermediate care units
  • Collaborates with bed control nurse to complete administrative tasks associated with inpatient transfers including but not limited to data collection of patient demographics, insurance, guarantor, information. May work with patient, family members, referring institution and/or other agencies to obtain accurate demographic and registration information
  • Carries out direction from Bed Control Nurse, Central Flow Unit and Capacity Transfer Center Leadership to prioritize/re-prioritize patients on an as needed basis and especially when operating in Code Help status
  • Monitors real-time BMC census; communicates with patient care units to ensure correct admission, discharge, and transfer information is reflected in Epic
  • Utilizes a call center type telephone system and maintains multiple lines for internal and external customers including but not limited to BMC leadership, physicians, nurses and patients; always communication in a courteous and professional manger while maintaining strict patient confidentiality
  • Participates in daily bed huddles to assess occupancy/capacity needs as well as convey physician care team status and share essential operational information needed to ensure appropriate patient placement
  • Utilizes standardized hand off tool to reduce avoidable errors, streamline essential information, and avoid a disruption of bed and care team assignment during change of shift
  • Serves as mentor for new employees and serves as an active participant in training new team members.
  • Answers multi-telephone lines in a polite and courteous manner, dealing with all internal and external customers looking for patient information, while maintaining patient confidentiality.
