About The Position

Antelope Valley Hospital's (AVH) Patient Placement Coordinator (PPC) is responsible for expediting patient admissions from various sources, including the Emergency Department (ED), Direct Admits, PACU, and Cath Lab, while focusing on Patient Throughput targets with clinical oversight. This role requires the ability to screen for level of care appropriateness and any special requirements around bed requests, such as isolation, custody, dialysis, and patient safety. The PPC serves as the point person for appropriate patient placement, leveraging clinical expertise to understand the hospital's capacity at all times and working closely with Bed Control and the House Supervisor. The PPC collaborates with Bed Control staff to review patient history and current medical needs, directing them on the appropriate bed placement based on current and historical data. Essential duties include demonstrating clinical knowledge to make informed decisions for specific patient populations, ensuring appropriate patient placement, and maintaining effective communication with the House Supervisor, Bed Control, and Charge Nurses. The PPC is also responsible for tracking updates and downgrades on nursing units, reviewing admission and transfer documentation, and ensuring compliance with bed placement policies and protocols. In addition, the PPC monitors patient movement throughout the hospital, maintains knowledge of discharges to assist with bed availability, and coordinates with various departments to ensure timely admissions and transfers. The role requires a high degree of flexibility and the ability to adapt to new situations, as well as the capability to handle problems collaboratively with the House Supervisor. The PPC also contributes to quality improvement initiatives and risk management activities, ensuring patient safety and confidentiality in all processes.
