About The Position

The position involves observing patients to ensure their safety and actively preventing self-harm or harm to others. Team members are expected to embody Prisma Health's values: Inspire health. Serve with compassion. Be the difference. The role requires constant 1:1 observation of assigned patients, documenting and reporting any risks or changes according to policy. Safety sweeps are conducted for patients at risk for self-harm, and the observer must ensure that the patient is not unattended, engaging assistance from unit staff or Security if necessary. The observer will also assist with meeting the patient's needs, including turning, positioning, feeding, ambulation, and transfers, while maintaining constant observation during bathing, toileting, and sleeping. Other duties may be assigned as needed.

Requirements

• High school diploma or equivalent preferred.
• No previous experience required; previous experience in a healthcare setting preferred.
• CNA or PCT certification preferred.

Responsibilities

• Provides constant 1:1 observation of assigned patient.
• Documents and reports any risks, changes or problems observed according to policy.
• Completes safety sweeps for patients at risk for self-harm.
• Ensures the patient is not unattended and engages assistance if the patient tries to leave the floor.
• Gives and receives hand-off communication regarding the patient assigned to observe.
• Assists with meeting patient's needs such as turning, positioning, feeding, ambulation, and transfers while maintaining observation.
• Attends to the patient at all times during bathing, toileting, and sleeping.
• Performs other duties as assigned.
