Requirements

  • Completion of an accredited or approved program in nursing
  • Registered Nurse license issued by the state in which the teammate practices
  • Current certification in BLS
  • BSN required
  • Prior experience in transplant, nephrology, hepatology, or complex chronic disease management
  • Strong care coordination, triage, and patient education skills
  • Experience working in an outpatient specialty clinic setting

Responsibilities

  • Assist in managing patient flow, scheduling appointments, and preparing clinic rooms for consultations.
  • Coordinate with the medical team to ensure timely completion of diagnostic tests and procedures.
  • Maintain accurate documentation of patient visits and follow-up plans.
  • Serve as the primary point of contact for patients, answering questions and providing education about liver conditions and treatment options.
  • Guide patients through the care continuum, including referrals, insurance processes, and post-visit follow-up.
  • Identify and address barriers to care, such as transportation issues or medication access.
  • Facilitate communication between patients and providers, ensuring that patient concerns are promptly relayed and addressed.
  • Assist providers by gathering relevant patient history and coordinating multidisciplinary team meetings as needed.
  • Support providers in implementing care plans and monitoring patient outcomes.
