When a patient requires care from different facilities and/or practitioners they need a Patient Nurse Navigator to identify needs and remove any barriers to timely, appropriate care. Our Nurse Navigators guide the patient from diagnosis through treatment and act as the central point of contact for the patient and the entire medical team. WHAT YOU CAN EXPECT For Cardiac Oncology A didactic learning experience in a fast-paced environment. Collaborate with 3 Cardiology Oncologist, 4 APP to assist in the provision of quality, cost effective patient care. Data collection. Maintain detailed knowledge of transfer center regulatory requirements. Provides clinical resources guidance on where to seek a second opinion, treatment or testing that may not be available locally, and how to access reliable sources of information. Patient & physician Liaison Answers questions as appropriate and serves as liaison to the physician for additional questions and concerns. Ensures any health information provided is accessible, such as securing documents printed in the patient’s own language or arranging for certified medical interpretation, either in-person or by phone/videoconference. A member of an interdisciplinary team to assess, secure, and coordinate appropriate services through the continuum of care. Educates patients about their disease, prescribed treatment, and side effects of treatment and how to manage them. Monday-Friday hours of clinic 8:00 am-5:30 pm No weekend, no holiday, no call
