About The Position

The Patient Navigator at St. Bernardine Medical Center plays a crucial role in providing outreach, education, and support for patients diagnosed with HIV, Hepatitis C, and Syphilis. This position focuses on improving health outcomes and quality of life for these patients by facilitating access to care, coordinating treatment plans, and advocating for necessary resources. The Patient Navigator will work closely with patients, healthcare providers, and community resources to ensure continuity of care and address barriers to treatment.

Requirements

  • At least two years of experience in a healthcare or social service setting, including community outreach and case management.
  • Accredited Associate's Degree with an emphasis in social services, psychology, or a related health field.
  • Strong knowledge of community resources in San Bernardino County and surrounding areas.
  • Excellent communication and interpersonal skills.
  • Ability to collaborate with interdisciplinary teams.
  • Strong computer literacy skills, including Microsoft Office and Google applications.
  • Knowledge of HIPAA guidelines and PHI privacy requirements.

Nice To Haves

  • Familiarity with medical and public programs such as Medi-Cal and Covered California.
  • Experience in providing application assistance for public benefit programs.

Responsibilities

  • Provide outreach, education, and coaching for patients diagnosed with HIV, Hepatitis C, and Syphilis.
  • Link patients to appropriate care and resources to improve health outcomes.
  • Document patient interactions and maintain accurate health data in a secure system.
  • Coordinate plans of care, including appointment reminders and transportation arrangements.
  • Advocate for culture change within the hospital and community regarding patient care.
  • Identify and minimize socioeconomic and environmental barriers to care.
  • Facilitate communication between patients, SBMC staff, and community health providers.
