The Patient Navigator at Pinnacle Fertility plays a crucial role in guiding patients through their fertility treatment journey. This position focuses on providing compassionate support, coordinating care, and ensuring that patients have a seamless experience as they navigate their path to parenthood. The role requires strong customer service skills and the ability to work collaboratively with a multidisciplinary team.

  • Serve as the primary point of contact for patients, guiding them through their treatment plan and providing education on procedures, testing, and follow-up care.
  • Coordinate and schedule patient appointments, tests, and referrals to ensure a smooth continuum of care.
  • Assist patients with understanding their medical care, insurance coverage, and available financial resources.
  • Communicate with healthcare providers, clinical staff, and administrative teams to ensure seamless patient care coordination.
  • Maintain accurate and up-to-date patient records, including documentation of interactions and care plans in the electronic medical record (EMR) system.
  • Provide emotional support and resources to patients and their families, addressing any concerns or barriers to care.
  • Participate in team meetings, case conferences, and quality improvement initiatives to enhance patient care delivery.
  • Educate patients on treatment protocols, self-care strategies, and medication usage.
  • Track patient progress, follow up on missed appointments, and ensure compliance with treatment plans.
  • Advocate for patients' needs and help resolve any issues or barriers to care.
  • High School Diploma required.
  • Bachelor's degree in healthcare, social work, or a related field preferred or a Certified Medical Assistant Certification (CMA) preferred.
  • Minimum of 2 years of experience in customer service, patient navigation, case management, or a related healthcare role.
  • Strong knowledge of healthcare systems, insurance processes, and patient advocacy.
  • Excellent communication, organizational, and problem-solving skills.
  • Proficiency in using electronic medical records (EMR) and other healthcare software.
  • Empathetic, patient-focused, and able to work collaboratively in a multidisciplinary team.
  • Bilingual skills are a plus.
