About The Position

The Patient Navigator will act as a liaison between patients, their families, and the healthcare team, primarily focusing on guiding patients through the complex healthcare system. This is accomplished by coordinating appointments, educating patients and families about their diagnoses and treatment options, advocating for their needs, and addressing potential barriers to care like transportation or financial concerns. This role will act as a dedicated point of contact to ensure patients receive timely and appropriate care throughout their healthcare journey.

Requirements

  • Minimum of 2 years' experience in a healthcare setting required; Medical Assistant experience preferred
  • Bilingual English and Spanish
  • Experience with Microsoft Office software (Outlook, Excel, Word), Insurance/Claims portals as well as Practice Management and Electronic Medical Record software.
  • Ability to work as part of a health care team; ability to embrace role in the customer service equation.
  • Highly organized
  • Communicates effectively in both oral and written formats
  • Must be able to visit multiple clinic sites as needed

Responsibilities

  • Care coordination: Coordinates scheduling and follow up of appointments, coordinating tests, and facilitating communication between different healthcare providers within the patient's care team both with internal and external clinicians and representatives.
  • Promote self-management support, including linking patients with educational services, obtaining preauthorization for tests and transportation, and facilitating connections to community-based organizations that address social determinants of health.
  • Referral coordination: Connecting patients with appropriate community resources, support groups, and specialists as needed.
  • Aid patients with referrals as needed to both clinical and non-clinical services.
  • Document referrals and follow up with clinical team members and patients as needed, including ensuring they have picked up their medications, attended scheduled appointments for diagnostics and specialists.
  • Barrier coordination: Assisting patients with transportation, childcare, housing, and other social determinants of health that may impact their care.
  • Patient Education: Explaining Plans of Care, referral processes, medication instructions and appointment adherence to patients and families.
  • Advocacy: Identifying and addressing patient needs, navigating insurance complexities, and advocating for necessary care.
  • Data collection and reporting: Tracking patient outcomes and contributing to quality improvement initiatives.
  • Prior authorization: Obtain prior authorizations as needed for medications and diagnostic testing
  • Complete other tasks as assigned
