About The Position

Overview Provides support for patients who need extra help navigating the complex health care system in various clinical settings. A strong patient advocate who will optimize the patient experience by serving as the liaison among the patient, healthcare providers, and the healthcare system. Supports a multidisciplinary team across multiple sections by serving as the liaison among the patient, healthcare providers and the healthcare system.

Requirements

  • Bachelor's degree in Social Work or Human Service field or the equivalent in education and experience required.
  • Excellent interpersonal, communication, and organizational skills required.

Nice To Haves

  • Two (2) years of experience interacting with patients in a clinical setting preferred.
  • Familiarity with data management and quality improvement is a plus.

Responsibilities

  • Identifies patients who require extra help in navigating the healthcare system.
  • Manages and coordinates patient centered care by acting as the liaison between patients and their interdisciplinary care team.
  • Provides administrative oversight to the section and may arrange scheduling between disciplines.
  • Facilitates communication and works to develop required programs and/or services.
  • Makes recommendations to the leaders to further develop the necessary clinic structure including organization of the materials and patient education information for the clinic.
  • Helps patients and families meet patient centered goals and seeks out appropriate resources.
  • Utilizes innovative strategies to advocate for patient needs and negotiates complex systems to remove barriers and limitations in accessing health care in all care coordinator areas including clinical, nutrition and psychosocial care.
  • Understands the patient journey and takes a systems level approach to improve the experience for all patients in the section.
  • Collaborates with leaders to develop process and procedures and identify future needs of the program.
  • Becomes an expert in the DHMC system, such that care can be managed efficiently by the coordinator with the patient and family.
  • Demonstrates excellent patient communication skills and empathy, with savviness to operationalize what is needed to improve the patient experience.
  • Promotes and teaches patient self-management skills to ensure long-term success with management by the interdisciplinary care team.
  • Collects and evaluates data to measure the success of the program and/or the patient's experience.
  • Interacts with various members of the care team to identify highest priority needs for improvement.
  • Performs other duties as required or assigned.
