About The Position

Simbioza Inc. is seeking a Patient Navigator to support patients across Florida as they move through the healthcare system. This is a part-time, telecommute / work-from-home position for candidates who reside in Florida and have completed recognized patient navigation training. This role supports patients through care coordination, insurance navigation, and follow-up services, with a strong focus on Medicare and Medicaid populations. The Patient Navigator works under the supervision of a Nurse Practitioner and supports Medicare-billable care management services. The Patient Navigator does not bill independently.

Requirements

  • Completion of a recognized patient navigation training or certificate program , such as: Healthcare Navigator Specialist Certificate (e.g., Eastern Florida State College or equivalent), or Susan G. Komen Navigation Nation training, or Comparable formal patient navigation education or credential
  • Minimum 1 year of experience in patient navigation, care coordination, or a closely related healthcare role
  • Strong working knowledge of Medicare and Medicaid
  • High school diploma or GED
  • Experience using EHR systems and basic scheduling tools
  • Strong communication and organizational skills
  • Ability to work independently in a remote environment
  • Must currently reside in Florida and be authorized to work in the United States
  • For patient navigation roles supporting Florida patients and programs, residency in Florida is the norm and is required.
  • Level II background check required

Responsibilities

  • Guide patients through appointments, referrals, and follow-up care
  • Support patients with Medicare and Medicaid eligibility, benefits, and coverage questions
  • Provide clear, patient-centered education and next steps
  • Coordinate with providers, clinics, and community resources
  • Identify and help resolve barriers to care
  • Document patient interactions and care coordination activities in the EHR
  • Communicate with patients via phone, email, and telehealth
  • Maintain HIPAA compliance and patient confidentiality
