About The Position

The Program Coordinator/Patient Navigator will be employed to administer a patient navigation intervention to guide participants to cancer screening and by identifying resources to overcome barriers to screening. The Program Coordinator/Patient Navigator will report to the Associate Director of Hollings Cancer Center Office of Community Outreach and Engagement (COE). The Program Coordinator/Patient Navigator will also administer a study eligibility screener to identify patients who are eligible to participate in the study’s navigation intervention and, where needed for a specific project, if a patient is eligible, administer the informed consent process to enroll the patient into the study. The Program Coordinator/Patient Navigator must be able to meet the eligible patients where they are (i.e., home, work, community center, libraries, etc.) if and/or when they do not have email access or do not know how to sign electronic form to obtain their signed consent forms. The Program Coordinator/Patient Navigator will collaborate with a nurse navigation team to achieve targeted patient outcomes. The Program Coordinator/Patient Navigator will identify and address barriers to cancer screening. The Program Coordinator/Patient Navigator will bridge the referral to outpatient providers with the patient to address issues and questions. The Program Coordinator/Patient Navigator will be responsible for study documentation and reporting patient navigation processes and outcomes in relation to the study participants.

Requirements

  • A bachelor's degree and one year relevant program experience.

Responsibilities

  • Recruits and coordinates the screening process with the health care team to achieve targeted patient outcomes and ensure a successful transition to the next stage of the treatment process, if any.
  • When applicable for a particular project, administers a study eligibility screener to identify patients who are eligible to participate and, if a patient is eligible, administers the informed consent process to enroll the patient into the study.
  • Provides patient navigation services to guide the enrolled study participants to cancer screening in collaboration with the health care team to achieve targeted patient outcomes.
  • Identifies and address barriers to cancer screening.
  • Bridges the referral to outpatient providers with the patient to address issues and questions.
  • Documents and reports patient navigation processes and outcomes in relation to the study participants.
  • Demonstrates current knowledge of community and other resource networks for planning/provision to meet patient needs.
  • Provides, connects, and educates patients and caregivers of needed resources.
  • Advocates for resources within the hospital and community to meet each patient’s needs.
  • Educates patients and families on lung cancer screening to enhance knowledge and increase positive treatment outcomes.
  • Integrate interventions into the interdisciplinary treatment plan; document in REDCap and communicates interventions in a timely manner.
  • Advocates for patient and family empowerment and right to self-determination regarding their healthcare.
  • Develops partnerships and function as a liaison with outside agencies, patients, the interdisciplinary team, and other members of the health care system to improve the continuum of care.
  • Collaborates with team members to enhance and update the navigation plan as needed.
  • Completes, reviews and addresses the barriers that may impact patient care.
  • Travels, when applicable, to federally qualified health clinics, other medical clinics, churches, and other community outreach sites to provide cancer education to community residents and patients.
  • Provides data analytic support to the Associate Director of Community Outreach and Engagement.
  • Provides preliminary analysis of data related to the MUSC HCC Office of Community Outreach and Engagement projects.
  • In collaboration with the Associate Director, develop and coordinates research manuscripts for submission to scientific peer reviewed journals.
  • Prepares summaries of the research literature that are relevant to the topic and scope of each manuscript.
  • Identifies and promotes individual and departmental goals that align with the assigned department and hospital’s strategic initiatives.
  • Identifies and promotes individual and departmental goals that align with Community Outreach and Engagement, as well as MUSC HCC strategic initiatives.
  • Identifies opportunities for process improvement.
  • Engages team members to identify optimal solutions to improve patient and community satisfaction and/or service delivery.
  • Participates in ongoing educational training and literature review to maintain the level of competence needed to perform job functions.
