Onboarding Process: Please be advised that candidates must successfully complete a background check and pre-employment health screening which includes a drug screen. Position Summary: Under general supervision, provides navigation assistance to patients receiving care in assigned areas. Identifies patients with specific diagnoses and connects those patients to coordinated care management. May provide referrals, appointment scheduling, and/or life-style education to patients as specified in program procedures. Educates patients on care management options in accordance with policies and procedures. Maximizes the number of individuals receiving access to primary care. May elicit patient consent to enroll in primary care-based ACO and Case Management programs. Communicates with patients about ACO and Case Management programs and model while offering referrals as needed and as specified in program. Maintains patient confidentiality and adheres to HIPAA laws. Provides excellent customer service. Collaborates and cooperates with care team members to ensure consistency and quality of care and promotes productive employee relations among staff members.

Requirements

  • High school diploma or equivalent
  • 1 year related experience in an outpatient clinic or hospital setting, including 6 to 12 months data entry experience

Nice To Haves

  • 1 to 2 years of work experience in insurance, medical records, billing, or related area
  • Medical Assistant certification
