About The Position

North American Medical Associates is multi-specilaty organization providing healthcare services across states of New Jersey, Pennsylvania, Delaware and New York. We take pride in providing advanced technology for our patients with chronic pain, injuries, and other ailments. Under indirect supervision, works closely with a specific patient population to improve patient experience through navigation, education, communication and resolution of barriers. May work with a multidisciplinary team including internal and external healthcare providers, administrative staff, and a wide range of referred services as indicated by the patient, care team, and/or needs assessment(s). Patient navigator will guide patients during their healthcare needs.

Requirements

  • High school diploma or GED; at least 1 year of experience directly related to the duties and responsibilities specified.
  • Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis.
  • Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community.
  • Knowledge of referred services and resources.
  • Ability to effectively interpret needs and act as an advocate for a specified client population.
  • Knowledge of available patient support programs, practices, and services.
  • Skill in the use of personal computers and related software applications.
  • Analytical and problem solving skills.
  • Ability to communicate complex information in a manner easily understood by patients.
  • Ability to work both independently and in a team environment.
  • Knowledge of advocacy principles and processes.
  • Skill in accessing internet information services.

Responsibilities

  • Based on the needs of the patient, provides navigation across the continuum of care, including diagnostic, treatment, and support services to address all aspects and phases of a specific health journey.
  • Assists patients with accessing services designed to support range of treatment determinants.
  • Helps patients navigate and address barriers to care.
  • Provides patients access to appropriate supports.
  • Provides resources and information to patients and families through established sources.
  • Supports patient-led decision-making and communication.
  • Provides accurate information and support appropriate to the patient with sensitivity to language and cultural preferences.
  • Establishes, maintains, and updates files, databases, records, reports and/or other documents.
  • Maintains accurate and complete patient files.
  • Participates in continuing professional development programs as appropriate.
  • May provide comfort and support to patients.
  • Performs miscellaneous job-related duties as assigned.
