The patient navigator RN provides patient-centered, trauma-informed medical case management to individuals across the HIV care continuum, ensuring timely access to medical services, treatment adherence, and continuous engagement in care. Also known as the RN Medical Case Manager (RNMCM), this position works as the lead of an interdisciplinary team to promote optimal clinical outcomes, viral suppression, and continuity of care. Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson. Conduct medical intake assessments for new HIV diagnoses, within 24 hours and an appropriate time for transfers Coordinate access to medical care, labs, medications, specialty services, and supportive programs. Provide education on HIV principals such as medication adherence, U=U, safer sex, etc Support rapid start of HIV Treatment Advocate on behalf of clients and monitor service utilization Engage clients every 6 months minimum Participate in chart scrubbing, pre-visit planning, and lead care teams Planning and execution of long-term injectables for prevention and treatment.
