About The Position

Patient Navigator II oversees the Kidney Transplant patient waitlist and coordinates appointments for patients awaiting kidney donation. POSITION SUMMARY: The Patient Navigator coordinates all administrative aspects of care for patients. The Patient Navigator, in collaboration with the clinical treatment team, patient and patients’ family/significant others work together to design a treatment and appointment calendar. The Patient Navigator also arranges interpreter services for all appointments, when necessary, schedules transportation as needed, and reviews the calendar regularly with the patient and the treatment team to ensure that the patient attends all appointments. The Patient Navigator also tracks all patient activity in the database and makes appointment reminder calls to ensure patients attend all appointments. The Patient Navigator is knowledgeable about community resources, including educational, social and emotional support services, available to cancer patients and connects patients with such when s/he identifies a need, either directly with the patient or through collaboration with the treatment team. Requirements

  • Bachelor’s Degree (or equivalent combination of education and experience, which provides proficiency in the areas of responsibility) is required.
  • Individual must have experience (paid or volunteer) working with families of various backgrounds.
  • Must have some familiarity navigating community resources and systems.
  • Knowledge of health care systems and community resources available.
  • Proficiency with standard software programs used in healthcare (such as MS Word, Outlook) and web browsers, patient registration systems and electronic medical record.
  • Exceptional organizational skills required.
  • Ability to multi-task and work under time pressures.
  • Ability to communicate well both verbally and in writing with other members of the clinical team.
  • Self-directed and independent.
  • Must be a self-starter and possess excellent customer service skills.
  • Demonstrated critical thinking and problem solving ability.
  • Knowledge of medical terminology

Nice To Haves

  • Experience within the assigned department working with patients is preferred, but not required.
  • Bilingual or multi-lingual skills (beyond that of English) appropriate to the patient population served (Spanish), is preferred.

Responsibilities

  • Works closely with the clinical staff to identify patients appropriate for participation in the Patient Navigation Program.
  • Understands the multitude of services required to provide a comprehensive treatment plan for each patient.
  • Tracks all patient information in the database required for the program.
  • Reviews medical information and coordinates with the medical team to understand each patient’s program.
  • Develops an appointment calendar for each patient with the assistance of the nurse and physician.
  • Meets with patient and family/significant others to review program calendar and services.
  • Works with medical records and the referring clinician to obtain all appropriate patient information.
  • Facilitates the flow of information among patient, clinicians and other services.
  • Interacts with all members of the team to facilitate the treatment plan.
  • Facilitates schedules for testing.
  • Works with clinical staff to ensure the patient and family understand all preparations required for procedures.
  • Functions as the liaison to the clinic team and is responsible for patient and family satisfaction
  • Interfaces with all departments to ensure efficient and effective working relationships.
  • Identifies system deficiencies and seeks to fill those gaps in collaboration with the Manager.
  • Has familiarly with community resources and shares them with patients and staff.
  • Develops and fosters relationships with clinical staff to become a resource for patients seeking research studies.
  • Represents Patient Navigation Services at local networking meetings.
  • Seeks guidance when necessary.
