About The Position
EvergreenHealth is seeking a Patient Navigator to join the Eastside Health Network team! This position is mostly remote with a requirement for monthly onsite meetings. The Patient Navigator (Health Resource Representative) is responsible for education and promotion of health-related services to the EvergreenHealth community and potential clients/patients through telephone, mail, and e-mail contact. The role also involves facilitating physician referrals, services, and class registration for EvergreenHealth and all other business contracts served. The Patient Navigator may serve as a healthcare information representative for unique service-line programs affiliated with EvergreenHealth, including accountable care networks where high-level benefit plan information, services, and primary care provider (PCP) information and scheduling are facilitated. Additional duties may be assigned as needed.
