Be a trusted guide for patients as they navigate care during and beyond cancer Survivor Healthcare is an innovative virtual telehealth clinic dedicated to improving the quality of life for people living with and beyond cancer. We partner with oncology practices to deliver comprehensive survivorship and supportive care, with a proven track record of improving patient outcomes and experiences. Our multidisciplinary model integrates oncology, occupational therapy, mental health, and nutrition to deliver compassionate, evidence-based care in a virtual-first environment. The Role As a Patient Navigation Assistant, you’re often the first voice our patients hear. You help them feel supported, prepared, and confident as they access virtual care. You work closely with Oncology Patient Navigators and clinicians to coordinate visits, answer questions, and ensure a smooth, reassuring care experience.

Responsibilities
- Support patients via phone and portal messages with warmth and empathy
- Schedule and coordinate virtual visits with navigators and clinicians
- Help patients prepare for telehealth, including setup and basic troubleshooting
- Assist with intake forms, insurance information, and documentation
- Ensure patient interactions and visit prep are accurately recorded in the EMR
