About The Position

The Patient Logistics Coordinator plays a crucial role in supporting the Medical Director of Emergency Medicine by overseeing Transfer Center operations, patient placement, and ensuring efficient patient flow. This position is designed to eliminate barriers in patient transfers and placements, working collaboratively with various healthcare staff to enhance patient care delivery.

Requirements

  • Minimum 3 years experience as EMT-P.
  • Minimum 2 years interfacility transport experience.
  • Proficient in Microsoft Office.
  • Associates Degree or higher required.
  • EMT-P NRP certification required.
  • CC-P or FP-C certification preferred.

Responsibilities

  • Provides direct oversight of each incoming transfer, intervening for any delays to ensure efficient transfer and appropriate service is advised of incoming transfers.
  • Escalates issues to the Medical Director of WMCHealth Emergency Medicine as appropriate.
  • Assists with triage of transport priorities for incoming transfers in collaboration with the Medical Director.
  • Helps patient placement coordinators in the placement of elective and emergent admissions and transfers according to clinical standards and guidelines.
  • Clinically knowledgeable of disease entities and epidemiological issues affecting patient placement.
  • Assists with the review of blocked beds and appropriate admission and level of care criteria.
  • Tracks bed availability and maintains appropriate logs/records.
  • Performs walking rounds on nursing units to identify and discuss barriers to discharge and confirm available beds.
  • Works directly with patient placement coordinators to monitor bed availability and pending discharges.
  • Assists with the distribution of transfer patients across the network.
  • Acts as a resource to patient placement staff regarding the type of patient each unit can accept using policies and procedures as a guide.
  • Regularly communicates with and provides status reports to the Medical Director regarding capacity needs and potential delays.
  • Performs quality assurance/improvement projects and data analysis.
