About The Position

The Patient Liaison plays a crucial role in providing direct care to patients by delivering medications on-site and ensuring timely medication refills. This position serves as a vital link between medical providers and patients, facilitating communication and addressing patient needs effectively.

Requirements

  • CCMA certification required.
  • At least 2 years of direct patient care experience.
  • Strong written and verbal communication skills.
  • Highly organized and able to prioritize tasks.
  • Strong work ethic and initiative.
  • Patience, positivity, and adaptability.

Nice To Haves

  • Bilingual in English/Spanish is a plus.
  • Familiarity with software applications related to the field.
  • Basic computer knowledge including email etiquette.
  • Professional phone and customer service demeanor.

Responsibilities

  • Delivering treatment to on-site patients and monitoring them after they self-administer.
  • Enrollment of patients in on-site medication programs.
  • Daily count of CDS (Controlled Dangerous Substances).
  • Obtain and monitor vital signs.
  • Coordinate lab services including urine drug screens.
  • Maintain medical supplies.
  • Act as a liaison between medical providers and patients to address patient needs.
  • Crisis intervention for on-site patients and those calling the facility.
  • Assisting with prior authorizations.
  • Completing and submitting all necessary monitoring paperwork within 7 days to patients receiving on-site services.
  • Documentation within 48 hours of service provided.
  • Ensuring that patients receive medication refills within 3 business days of calling.
