About The Position

The Center for Operational Resource Efficiency (CORE) works with patient care areas, support services, administration, and others to implement strategies to ensure seamless patient care, capacity management, efficient patient flow, improved access and quality care. Goals of CORE include reducing missed transfer opportunities, increasing targeted volumes, reducing unnecessary length of stay (LOS), and placing the right patient in the right setting at the right time. The department defines the analytics required to predict and drive decisions in real-time. Metrics are tracked and outcomes reported to sustain an environment of continuous improvement Job Description PRIMARY FUNCTION: The Patient Flow Nurse is responsible for continuity of operations within the facility by overseeing all patient admissions, discharges, and transfer activity, while monitoring capacity both currently and prospectively across the TJUH, Inc. Division. Facilitates continuous throughput of patients from all points of entry by utilizing effective verbal communication, swift clinical decision making and problem-solving skills. This includes coordination and communication with key personnel and departments, including Jefferson physicians, nursing personnel, Environmental Services, Patient Transport, JeffSTAT, and the Jefferson Health Transfer Center. The Patient Flow Nurse operates in a busy, high-stress environment, and must be able to make stressful decisions promptly. The Patient Flow Nurse Manager must be adaptable individual capable of building relationships and managing competing priorities. The Patient Flow Nurse is accountable for defined patient flow quality and safety key performance indicators (KPIs), e.g. ED and PACU boarding, clinical place assignment/placement times, wait times in ED and PACU and adherence to clinical placement guidelines/ unit-based admission criteria

Requirements

  • BSN- required
  • Registered Nurse (RN) required
  • Advanced Cardiac Life Support (ACLS) Certification required
  • 3-5 years In a related area of clinical practice required
  • MS Office (Outlook, Word, Excel, PowerPoint, etc.), EMR Skills and Knowledge
  • Ability to develop effective relationships with a range of stakeholders internally and externally.
  • Critical thinking skills.
  • Risk management strategies
  • Problem solving, relationship building, interpersonal, communication, conflict resolution

Nice To Haves

  • MSN or healthcare related area- preferred
  • Registration with related professional association preferred
  • Lean Six Sigma and/ or Project Management preferred
  • 1-2 years Supervisory or Charge Nurse experience preferred

Responsibilities

  • Maintains patient throughpout and continually evaluates bed placement progress according to the institution’s Patient Flow Plan.
  • Provides clinical guidance to departmental personnel on where to place patients in accordance with unit-based admission and clinical placement criteria.
  • Establishes and directs plan for patient placement of all admissions and transfers.
  • Collaborates with medical staff and nursing personnel in all departments to optimize patient flow throughout the facility.
  • Monitors centralized care progression and quality and safety alerts.
  • Maintains awareness of staffing levels in key departments, such as Nursing, EVS, Patient Transport, JeffSTAT, and other departments that might impact patient flow.
  • Acts as the Charge Nurse in the absence of an on duty Manager or Supervisor.
  • Targets and assigns beds to patients from all clinical departments, direct admissions, and external transfers based on defined workflow.
  • Acts on care progression and quality and safety alerts based on clinical guidelines, protocols and processes.
  • Facilitates the Daily Bed Meeting and leads efforts to problem solve during high census periods and develops action plans.
  • Sets the priority for EVS and Patient Transport requests to optimize patient flow and reduce wait times.
  • Notify Facilities Services of maintenance requests for bed blocks.
  • Performs duties in an independent manner based on guidelines set by supervision.
  • Participates in special projects or other duties as assigned.
