The Patient Flow Facilitator (PFF)-RN is a highly skilled licensed registered nurse that is competent to provide care to patients admitted to Loma Linda University Medical Center (LLUMC). The PFF nurse does not have a physical home unit, but instead, functions out of a home department and readily floats to all areas of the hospital with a primary focus of patient flow. This includes, but is not limited to, maintaining specified level of care while transporting patients with admission orders from the emergency department to the unit, transporting patients within LLUMC units and assisting with expediting admissions, transfers and discharges. Based on business needs, may be required to serve in the hospitality/discharge lounge or accept a full or partial patient assignment on a unit. Functions in a patient-centered care model of practice and performs the nursing process independently. Serves as an exemplary role model in patient care delivery methods and consistently demonstrates an advanced level of clinical practice, communication, and documentation. Facilitates the empowerment of the nursing profession and excels in implementing new knowledge, innovations and improvements in practice to enhance the patient experience and clinical outcomes. Applies theoretical concepts to clinical practice and continually expands knowledge base and clinical skills with a focus in the pediatric population. Utilizes interpersonal skills to maximize excellence in customer service, safe patient care, and professionalism. Contributes to a work environment of caring and cooperation among a culturally diverse workforce and patient population. Performs other duties as needed.
