The Patient Flow Coordinator (PFC) functions as a hybrid and will extend both Patient Flow Coordinator (PFC) and case management (CM) capabilities into off shift hours. The primary focus for this job will be to manage Fox Chase Transfer Center (FCTC) operations; other responsibilities include finalizing case management details for evening and weekend discharges and covering triage phone calls. Responsibilities to the FCTC are focused on being the first point of contact for outside patient transfers and internal admissions. The PFC puts the transferring physician/FCCC clinic/DRU physician in contact with the appropriate FCCC accepting physician and actively manage beds throughout the institution; finalizes case management-related disposition, serves as a resource for transfer-related insurance issues and answers incoming triage calls.
