About The Position

Collaborating with the Clinical Administrative Supervisors (CAS) and Director of Nursing (DON), the Patient Flow Coordinator (PFC) performs duties to assist with the information needed to improve patient flow in real time by communicating directly with areas providing services. The PFC will work under the direction of the CAS to improve patient flow through internal and external transfers and admits. An Integral component consists of strong interpersonal skills with an emphasis on collaboration with other health care facilities, support services, physicians, patients, and patient’s families. Technical tasks involve utilizing computer skills efficiently on an ongoing basis. The PFC performs clerical duties including data collection and input for patient transfers and admits. Financial performance measures include review of patient charts for completion to ensure accurate and timely processing of information. The PFC uses capacity management systems and protocols to support through put, including: Assigning patients rooms, monitoring room availability and turnover, and maintains electronic bed board. The PFC also serves as a resource for staff and management. Assist with coordination of units staffing and utilization.

Requirements

  • Minimum two year degree and/or five years of experience in healthcare systems and services.
  • Proficiency with Hospital Information Systems.
  • Good verbal and grammatical skills required.
  • Good customer service experience.

  • Communicates effectively and courteously with providers, outside healthcare facilities, and interdepartmentally to obtain and share information necessary for patient transfers and admissions.
  • Documents accurately and clearly each telephone encounter with the transferring facilities in the electronic intake board.
  • Obtains, verifies and accurately records patient demographics, diagnosis, procedures, transferring and accepting providers, and special patient considerations.
  • Appropriately requests guidance to ensure that patient data is inputted correctly.
  • Transmits all requests for specialty services to appropriate providers
  • Utilizes proactive tactics to most efficiently utilize bed capacity and correlating staffing.
  • Maintains current knowledge of bed capacity, unit staffing, and organizational flow processes
  • Assigns appropriate patients to beds
  • Maintains open communication with providers and managers on impending transfers and bed availability

  • Adheres to all regulatory/safety policies and requirements.
