About The Position

The Patient Flow Advisor works in the Patient Flow Center triaging phone calls and overseeing the daily processes related to bed placement, patient transfers, and admissions. The Patient Flow Advisor is responsible for overall bed management and throughput by working with the unit-based Care Facilitators. The Patient Flow Advisor works closely with the Patient Access Liaison to ensure timely registration, insurance verification, precertification and general administrative tasks related to the admissions process. The Patient Flow Advisor works to find accepting physicians and also to obtain admission orders at the earliest possible time for patient admissions.

Requirements

  • Current Registered Nurse in the State of Ohio.
  • BSN preferred.
  • Prior Med Surg experience (highly preferred).
  • Certification in area of case management (preferred).

Responsibilities

  • Triage phone calls and oversee daily processes related to bed placement, patient transfers, and admissions.
  • Manage overall bed management and throughput in collaboration with unit-based Care Facilitators.
  • Work closely with the Patient Access Liaison for timely registration and insurance verification.
  • Facilitate communication between nursing units and physicians for seamless admissions.
  • Answer telephone inquiries with appropriate introduction of self and service.
  • Facilitate patient transfers to and from the Medical Center.
  • Assist with direct admissions and transfers from other facilities.
  • Communicate with nursing units to gather necessary information for optimal patient placement.
  • Obtain required patient identifying data for admissions.
  • Facilitate 3-way calls with transferring facilities and physicians.
  • Assign patient admission status based on information obtained.
  • Enter GMLOS and Target LOS in StatCom based on patient information.
  • Create pending bed requests for notification to Care Facilitators.
  • Participate in alert activation processes for STEMI alerts.
  • Perform triage interviews to assist in physician selection based on guidelines.
  • Answer Rapid Response Line and notify Nursing Supervisor or Response Team.
  • Maintain confidentiality of all interactions.
  • Complete other duties as assigned by the Manager.
