About The Position

HCHC is a dynamic community health center that offers a health care delivery environment that ensures all members of our community have access to high quality primary care, dental, and behavioral health services. We also provide medication assistance support to ensure our patients have access to the lowest cost for their medications, transportation assistance to and from appointments if needed, community resource connections to close gaps with social determinants of care, integrated care models including targeted chronic disease management support with our pharmacy team and much more. We assist patients in getting access to medicaid as well as other insurance options using trained professional staff who serve in these roles. Hours of operation: Monday-Friday 7:30AM-6:00PM, Saturdays 8:00AM-12:00PM.
