About The Position

CareBridge Health, a member of the Elevance Health family of companies within the Carelon business, exists to enable individuals in home and community-based settings to maximize their health, independence, and quality of life through home-care and community-based services. The Patient Enrollment Specialist – Transitions of Care is responsible for new and existing patients. This role operates on a rotating schedule of four (4) 10-hour shifts per week, from 8:00am - 7:00pm CST, and requires working every other weekend (Saturday and Sunday). The initial training schedule is Monday – Friday, 8:00am – 5:00pm CST for the first two weeks. This is a virtual role, allowing associates to work virtually full-time, with the exception of required in-person training sessions, promoting flexibility and autonomy. Candidates must reside within a reasonable commuting distance from the posting location(s) to be considered, unless an accommodation is granted. Associates are expected to multi-task, handling calls, texts, facsimiles, and electronic queues while simultaneously taking notes and speaking to customers. Key attributes include proficiency in maintaining focus during extended periods of sitting, handling multiple tasks in a fast-paced, high-pressure environment, strong verbal and written communication skills, attention to detail, critical thinking, problem-solving, empathy, persistence in resolving caller issues, and comfort with digital tools. The role has a structured work schedule with occasional overtime or flexibility based on business needs, including the ability to work from the office as necessary.

Requirements

  • HS diploma or equivalent
  • 1 year of experience in call center, sales, health care or customer focused environment; or any combination of education and experience which would provide an equivalent background
  • Ability to multi-task, including handling calls, texts, facsimiles, and electronic queues, while simultaneously taking notes and speaking to customers
  • Proficient in maintaining focus during extended periods of sitting and handling multiple tasks in a fast-paced, high-pressure environment
  • Strong verbal and written communication skills, both with virtual and in-person interactions
  • Attentive to details, critical thinker, and a problem-solver
  • Demonstrates empathy and persistence to resolve caller issues completely
  • Comfort and proficiency with digital tools and platforms to enhance productivity and minimize manual efforts
  • Ability to work from the office as necessary
  • New candidates in certain patient/member-facing roles to become vaccinated against COVID-19 and Influenza (unless an acceptable explanation is provided or prohibited by law)

Responsibilities

  • Reviewing and prioritizing patient census
  • Reaching out to patients to explain services and enrolling them into the appropriate programs based on the line of business
  • Providing support by addressing inquiries and scheduling the initial and annual appointments
  • Enters documentation into the designated systems
