This job handles patient population management, outreach, education, and connecting the patients to the organization's health network supporting limited network products, patient attributed shared savings programs, and place of service care initiatives. Stratifies patient populations, applies the appropriate engagement tactics to support the stratified population to close care gaps, promotes wellness screenings, and facilitates the completion health risk assessments (HRA). Tracks all activity and outcomes utilizing the internal HRA database as well as scheduling appointments in Epic.
