About The Position

This job handles patient population management, outreach, education, and connecting the patients to the organization's health network supporting limited network products, patient attributed shared savings programs, and place of service care initiatives. Stratifies patient populations, applies the appropriate engagement tactics to support the stratified population to close care gaps, promotes wellness screenings, and facilitates the completion health risk assessments (HRA). Tracks all activity and outcomes utilizing the internal HRA database as well as scheduling appointments in Epic. Requirements

  • High school diploma or equivalent.
  • 2 years experience working in sales, patient engagement, community education, community health or providing outreach programs.
  • Proficiency in using computers, software, and web-based applications.
  • Effective verbal and written communication skills and ability to present information clearly and professionally to varying levels of individuals throughout the patient care process.
  • Working knowledge of database programs.
  • Working knowledge of medical terminology.
  • Ability to travel throughout and between facilities.

Responsibilities

  • Handles direct outreach to targeted patients to meet identified goals and targets for the stratified population.
  • Handles data and project management including refreshing patient data as updates are made available.
  • Tracks activities, outreach efforts, and care results in the HRA database.
  • Participates in administrative duties to support program, participating in training and on-boarding new team members.
  • Stays current with healthcare related industry trends by participating in OLN classes as well as attending outside seminars and related reading materials.
  • Performs other related duties as required.
