About The Position

The Patient Educator (Health Coach) role at Sutter Health involves providing comprehensive health education and support to patients and employees. The position focuses on motivational counseling to enhance adherence to medical treatments and promote healthier lifestyle choices. The candidate will also assist in crafting training materials for program launch and handle patient inquiries related to the program. This role requires a strong presence at the Innovation Center in San Francisco initially, with potential for remote work after onboarding.

Requirements

• Bachelor's degree or equivalent education/experience.
• 5 years of recent relevant experience in health coaching or related fields.
• Experience coaching individuals in lifestyle changes, particularly for chronic diseases.
• Familiarity with social determinants of health and their impact on patient self-efficacy.
• Mastery of basic computer skills and knowledge of medical terminology.
• Excellent interpersonal, organizational, written, and verbal skills.

Nice To Haves

• Master's degree in Public Health, Nutrition, Exercise Physiology, or Social Work.
• Experience in developing educational materials and programs.

Responsibilities

• Provide comprehensive health education and support to patients and employees.
• Engage referred patients in motivational counseling sessions to facilitate adherence to prescribed medical treatments.
• Craft training and operational preparedness materials for program launch.
• Handle inbound patient inquiries specific to the program and devices.
• Develop and implement educational programs and events within the clinic and community.
• Support clinical teams in addressing individual patient needs via phone or in-person.
• Schedule and conduct initial intake sessions and follow-up coaching sessions with patients.
• Establish behavioral goals and personal accountability systems with patients.
• Reinforce patient learning by providing approved educational materials.
