We’re looking for a highly organized and empathetic Home Health Patient Coordinator to join our team and serve as a crucial link between our technology and the patients who need it most. In this role, you’ll be the primary point of contact for patients beginning their journey with Circadia’s in-home monitoring technology. You will be responsible for patient outreach and education, answering questions, and scheduling the installation of our monitoring devices with our field operations team. The ideal candidate has exceptional communication skills, a passion for helping others, and the ability to manage a high volume of outreach with professionalism and care. You should be confident working with digital tools, explaining new technology in a simple, supportive way, and ensuring every patient feels comfortable and informed.
