The Patient Centered Primary Care Home Coordinator (PCPCHC) is responsible for the successful execution, coordination, and management of the Patient Centered Primary Care Home (PCPCH) program.

  • Oversight of Patient Centered Primary Care Home (PCPCH) requirements including the understanding of PCPCH model, facilitates and participates in annual review and ongoing progress to meet the tier requirements.
  • Works closely with Director of Operations to Determine Project time frames, procedures for accomplishing project and lead others in the organization to make meaningful process improvements.
  • Make presentations to leadership or staff, ensure project documentation is accurate and ensure projects are completed on time and within scope.
  • Sets and track project milestones and accounts for unforeseen delays, then realigns schedules and expectations as needed.
  • Proactively seek opportunities to improve process or infrastructure and collaborate with peers for change.
  • Develop, document, and implement project communication tools and plans.
  • Maintain subject matter expertise on PCPCH requirements to drive program participation and promote development including tier advancement.
  • Participate on Quality Improvement and Risk committees to help develop and deploy aligned quality performance programs to drive better provider satisfaction and quality metric outcomes.
  • Facilitate a monthly Patient Focus group that focuses on projects from Quality Improvement/Risk committee.
  • Other duties as assigned.
  • High school diploma or equivalent.
  • Three years' experience working in health care setting.
  • Must be 21+ and possess a valid driver's license
  • Associate's degree preferred.
