The Patient Centered Primary Care Home Coordinator (PCPCHC) is responsible for the successful execution, coordination, and management of the Patient Centered Primary Care Home (PCPCH) program.
Oversight of Patient Centered Primary Care Home (PCPCH) requirements including the understanding of PCPCH model, facilitates and participates in annual review and ongoing progress to meet the tier requirements.
Works closely with Director of Operations to Determine Project time frames, procedures for accomplishing project and lead others in the organization to make meaningful process improvements.
Make presentations to leadership or staff, ensure project documentation is accurate and ensure projects are completed on time and within scope.
Sets and track project milestones and accounts for unforeseen delays, then realigns schedules and expectations as needed.
Proactively seek opportunities to improve process or infrastructure and collaborate with peers for change.
Develop, document, and implement project communication tools and plans.
Maintain subject matter expertise on PCPCH requirements to drive program participation and promote development including tier advancement.
Participate on Quality Improvement and Risk committees to help develop and deploy aligned quality performance programs to drive better provider satisfaction and quality metric outcomes.
Facilitate a monthly Patient Focus group that focuses on projects from Quality Improvement/Risk committee.
Other duties as assigned.
High school diploma or equivalent.
Three years' experience working in health care setting.
Must be 21+ and possess a valid driver's license
Associate's degree preferred.
