About The Position

The Patient Care Unit Coordinator assists patients with activities of daily living, provides basic nursing care to non-acutely ill patients and assists in the maintenance of a safe and clean environment. Functions under the direction and supervision of the RN. Under the supervision of the Nurse Director/ Manager or delegate, the Patient Care Unit Coordinator assists in planning, organizing, implementing and evaluating the activities occurring in the nursing unit. Performs clerical and receptionist duties and maintains the physical environment of the area. This position deals with a patient population ranging in age from adolescence to geriatrics with surgical/medical clinical focus. Job Description: DUTIES AND RESPONSIBILITIES 1. Performs all patient record activities and charting transcriptions with close attention to detail and accuracy. Ensures that complete information is filled in on all forms/requisitions, transfers, work order, dietary requisitions and keeps current census sheets. Delivers completed forms to the proper departments, as necessary. 2. Initiates chart forms for patient admissions, transfers, discharges and expirations; maintains patient’s charts in appropriate organizational and sequential order; 3. Identifies correct patient charts and reports before filing; verifies that all information is correct and takes corrective action when necessary (patient labels, face sheet, ID band, etc.). Ensures accuracy at all times. 4. Routinely places appropriate forms on charts, without reminders, (Ensures that all documents in record are properly numbered, dated and stamped with correct patient identification. Patient files are current, at all times. Assists with prep of pre-surgical charts. 5. Responds immediately to patient call lights. Notifies appropriate nursing personnel of patient requests. Promptly delivers mail, newspaper, messages and flowers to patients. 6. As required, notifies departments appropriate for task (repairs, discharges, order changes, dietary orders, etc.). 7. Obtains old medical records for patients upon their admission 8. Updates, pads, stuffs and thins “long stay” charts, as prescribed by policy, when necessary - needs no reminders. 9. Disassembles all charts of discharged patients regardless of the shift that the patient was discharged from the hospital. 10. Schedules appointments for patient examinations and treatments, makes arrangements for transportation to other facilities for same if necessary. These bookings are made in a timely manner so that patient’s hospital stay is not unduly prolonged. 11. Promptly notifies departments of STAT tests and treatments. 12. Demonstrates competence in functioning as a receptionist on the nursing unit and communicating with physicians, ancillary departments, patients and visitors (receives, greets and directs patients, visitors and hospital personnel providing accurate responses in a friendly, courteous manner; acts as a nursing station hostess, extending courtesies to the patients, visitors, families and staff). 13. Receives, places and transfers calls using appropriate telephone etiquette; identifies self and unit, handles telephone information requests with courtesy, accuracy and respect for patient confidentiality, delivers messages to appropriate person and forwards all information as required. 14. Remains composed, reassuring and able to follow direction during emergency situations, STAT requests and orders; functions as a calming influence and stabilizing factor on the nursing unit. 15. Promptly makes all arrangements for outside services, transfers, CAR scans, ambulance services, etc.; coordinates schedules for maximum efficiency. 16. Demonstrates the ability to adapt to changes in the workload as required, in view of patient census fluctuations and staffing levels; always uses time productively. 17. During each shift routinely inventories and orders unit stock on a weekly basis; ensures proper supplies are on hand at all times. Maintains a supply of admission charts on hand. 18. Demonstrates knowledge of documentation procedures (vital signs, intake and output, activity sheet, progress notes, etc.). 19. Demonstrates a thorough working knowledge of medical, surgical and pharmaceutical terminology; asks questions to clarify misunderstandings. 20. Demonstrates knowledge of importance of always identifying correct patient records, reports, charts, etc. 21. Provides accurate and timely information using utmost discretion and protecting confidentiality of information. 22. Possesses an awareness of hospital systems and procedures, location of all departments, treatment areas and other patient services. 23. Assists in the orientation of new physicians, nursing personnel and others as assigned, to the routines of the nursing unit; participate in on-the-job training of new Patient Care Unit Coordinator. 24. Regularly locates attending or consulting physician for RN’s, as required. 25. Demonstrates the ability to provide accurate information in a concise, informative manner, respecting patient’s right to privacy; places particular emphasis on the sensitivity of a patient’s chart. 26. Willingly accepts temporary reassignment to other units in order to meet the needs of the department. 27. Establishes and maintains a good rapport and cooperative working relationship with co-workers and supervisors in order to foster appropriate communications necessary for delivery of good patient care, knows all coworkers by name; responds to co-workers with concern and cooperation. 28. Informs Charge Nurse when leaving unit to perform errands. Plans time off floor for above purposes at less busy times for unit. 29. Coordinates the scheduling of diagnostic tests and procedures in an effective and efficient manner to eliminate the need for rescheduling and unnecessary delays; coordinates appointments with other departments. 30. Assists in the coordination of patient admissions, transfers and discharges to provide for a smooth operation and efficient flow pattern. 31. Always “reports off”, before leaving the unit 32. Willingly performs or offers to perform assignments that need to be completed even when they are outside the scope of routine responsibilities. 33. Regularly consults with other nurses in obtaining further information on patient care procedures; readily observes or assists others to obtain further knowledge 34. Seeks guidance and direction as necessary for successful performance of duties. 35. Attends all Joint Commission required in-services yearly 36. Attends mandatory scheduled staff and nursing assistant meetings. 37. Actively participates in scheduled in-service programs yearly; attends all mandatory in-service classes as assigned 38. Always assumes responsibility for reading and comprehending all posted minutes and memos. 39. Maintains annual mandatory certification as required. 40. Consistently demonstrates knowledge of work attendance schedule. 41. Demonstrates an ability to perform in an accurate and precise manner in crisis and emergency situations when time is essential under the direction of the RN. 42. Participates in hospital emergency and disaster drills; knows personal role in event of a fire while on duty.

Requirements

  • High School Diploma/GED; or C.N.A. certification; or equivalent related work experience required.
  • AHA Basic Life Support Certification or must be completed within 30 days of hire
  • Excellent organizational skills and ability to work cooperatively and effectively with others.
  • Experience with the use of computers preferred or familiarization with keyboard and typing skills preferred
  • Familiarization with medical terminology and excellent organization skills

Nice To Haves

  • Nursing Assistant Certification, prior LTAC or med/surg experience, or nursing clinical rotation, preferred.
  • MOAB and first aid training preferred
