Patient Care Technician
About The Position
The Patient Care Technician (PCT) will work on-site at DaVita Skilled Nursing Facility (SNF) dialysis dens, specifically at Northland Rehabilitation and Highland Rehabilitation in Kansas City, MO. This role is part of DSD Services, which aims to leverage DaVita's resources to develop and grow a new service line focused on providing dialysis care within SNFs, addressing the burden and costs associated with transporting patients. Ideal teammates are those with significant experience within DaVita, who are energized by a start-up environment, committed to improving patient lives, and adept at finding solutions. Teammates must be comfortable working autonomously and collaborating with cross-functional resources. In this position, the PCT is responsible for the complete ongoing care of each patient, ensuring their dialysis treatment remains on track. PCTs will work alongside dialysis RNs to deliver care, spending most of their day providing direct one-on-one patient care to ensure safe, comfortable, and hygienic dialysis treatment. DSD Dialysis care is a complex but fulfilling service line, requiring multitasking to meet the needs of the service, patients, and SNF partners while striving to meet or exceed clinical outcomes. The role demands autonomy and critical thinking, as PCTs provide comprehensive patient care in an independent setting, constantly monitoring patients before, during, and after dialysis treatment, including measuring and recording stats, patient observations, and hemodialysis machine setup.
Requirements
- High school diploma or equivalent
- MUST BE CCHT certified
- Basic computer skills and proficiency in MS Word and Outlook

Responsibilities
- Take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track
- Work in conjunction with dialysis RNs to provide dialysis care
- Spend the majority of your day in direct one-on-one patient care to provide safe, comfortable and hygienic dialysis treatment
- Meet and/or exceed clinical outcomes
- Multitask to meet the needs of the service, the patients and our SNF partners
- Provide comprehensive patient care in an independent setting
- Monitor patients before, during and after dialysis treatment including measuring and recording stats, patient observations, and hemodialysis machine setup
- Commit to improving patient health through clinical goal setting and quality improvement initiatives
