About The Position

As a Patient Care Technician (PCT), you will deliver essential direct patient care under the supervision of our clinical team. Leveraging your knowledge of basic patient care principles and medical terminology, you will perform a range of daily duties. Every day you will actively assist patients with personal care, mobility, and feeding, monitoring vital signs, documenting observations, and maintaining a therapeutic and organized patient environment. You will also work closely with the nursing team to prepare patients for procedures or transport. To be successful in this role requires a genuine passion for patient well-being, diligence in executing tasks, keen observational skills regarding patient conditions, and a proactive approach to continuous learning and professional development.

Responsibilities

  • Ability to take vital signs.
  • Assists with Activities of Daily Living. Cleansing bath, back/skin care, hygiene, care of dentures, hail and nail care, perineal care, shaving male patients
  • Assist patient with elimination. Colostomy care, perineal care
  • Provides infant care as needed including diaper changes, cord care and bathing.
  • Assist patient with meals, including feeding patients. Assist with completion of menus. Keep patients supplied with fresh drinking water. Measure and record oral intake and output.
  • Make beds. Tidy patients' room/area. Keep patients' equipment clean.
  • Simple Dressing changes, Annual competencies, Foley catheters - dailycare, Mouth suctioning-preparation for procedures.
  • Enters orders in designated computer system.
  • Acts as unit receptionist to physicians and visitors, Answers phone professionally and courteously.
  • Enters orders via designated computer system.
  • Reviews Physician's orders, following up as indicate. Notifies Licensed staff/Registered Nurse of new orders. Faxes/scans medication orders to Pharmacy.
  • Prepares chart for admits. Maintains chart in proper, complete order. Adds forms, as required. Places reports on charts. Reviews chart for signatures and completeness. Reviews orders for completion. Maintains and tracks all loose documents
  • Enters appropriate Intervention Order sets and interventions into electronic record
  • Keeps floor census up-to-date including, entering transfers to unit into Admitting module of Hospital Information System (HIS) and enters discharge into Admitting module of HIS.
  • Keeps floor stock supplies at proper level including, orders and stocks all forms, secretarial and miscellaneous supplies. Keeps general order of Nursing stations including, distributes mail.
  • Ensures that medical records are set aside for pick-up by medical records within 24 hours of patient discharge.
  • Answers all telephone calls within the first three rings stating department name, your name, title, and "How may I help You"
  • Answers call lights within a timely manner (the needs of the patient need to be addressed within 2 minutes) stating your name, title and "How may I help you" and directing the call to the appropriate personnel
  • Answers call lights, via the intercom system, referring patient needs to the appropriate personnel.
  • Run one strip per patient every shift and/or as needed for rhythm changes or dysrhythmias. Review dysrhythmias and clear appropriately at least every four hours as needed.
  • Monitors patient in appropriate lead for observance of ischemia, infarction, and ectopy. Record admission cardiac rythm within fifteen minutes of admission. Review and set alarm limits at beginning of each shift.
  • Respond to all indicated alarm situations by running strips immediately. Notify RN of all significant rhythm changes. Remain at console at all times unless relieved by monitor-trained personnel. Keep monitor station clean at all times.
  • Performs EKGs with appropriate training.
  • Notify charge and/or nurse manager of monitor malfunctions and enter work order into the computer. Track and maintain telemetry transmitters
  • Participates in performance improvement activities for the unit. Attends at least 50% of team meetings. Reads and signs meeting minutes. Attends all mandatory meetings or classes.
  • Observes patients who are in need of 1:1 observation by sitting in the patients room at all times.
  • Verbalizes the importance of constant visual observation and not leaving the patient at any time (including when the patient has visitors) unless charge nurse has coordinated a rest or meal break.
  • Documents on the Special Observation Check list every 15 minutes.
  • Completes a room search removing any inappropriate items.
  • Observes multiple patients when aggregated together as deemed necessary by nurse.
  • Relays patient needs to the nursing staff for prompt attention. Reports changes observed to the nurse.
  • Protects patient privacy under video surveillance during hygiene activities, treatments and when requested, as appropriate.
  • Assists the nurse caring for the patient(s) with hygiene, activities of daily living if appropriate.
  • Assists the nurse caring for the patient(s) with patient requests for water or food as directed when appropriate.
  • Provides conversation and socializing as a means of diversion when appropriate.
  • Protects the patient from injury by removing potentially harmful items or asking for assistance from the nursing staff.
  • Communicates effectively with members of the interdisciplinary treatment team, patients, families and visitors utilizing knowledge of and demonstrated skills related to interpersonal relationships and communication styles appropriate to the individual's uniqueness and age.
  • Provides a safe and healthy environment for patients, families, visitors and the interdisciplinary treatment team by utilizing knowledge of proper body mechanics and infection control standards.
  • Communicates in language appropriate for age.
  • Allows control and choices over situations and environment, as appropriate to individual's condition and situation.
  • Recognizes the importance of confidentiality and allows for privacy needs.
  • Recognizes signs of abuse/neglect potential.
  • Assisting with ambulation and transportation of patients when directed and it is appropriate.
  • Promote the patient's right and need for privacy and maintain confidentiality.
  • Provide emotional support for patients and their families by utilizing communication skills.
  • Seek and accept direction from staff RN, BC Clinical coordinator and Nurse Manager.
  • Reporting significant changes in patients' condition to the RN or Charge nurse immediately.
  • Participate in quality assurance activities.
  • Make rounds on patients and assist mother with checking infant voiding, meconium stools, and feedings. Assist mother with diapering of infant.
  • Sets table for delivery for physicians and one after shift for emergencies
  • Attend deliveries and assist nurse as necessary. Helps with cleanup patients and placing mother back in bed following delivery.
  • KEEPS FLOOR STOCK SUPPLIES AT PROPER LEVEL. Orders and stocks delivery and other miscellaneous supplies. Maintains par cart. Collaborates with unit staff to determine and order other unit supplies. Stocks and maintains supplies in rooms and on delivery carts. Assists in putting away stock when delivered by Warehouse.
  • Enter patient name and medical record number into the QS System before placing patient on monitor. Assists in ensuring that Newborn Screening forms are complete, logged in and sent out in a timely manner.
  • Ability to complete all paperwork involved with a C/S admission as well as an admission to NICU. Assist as needed in NICU.
