About The Position

Primary Purpose: To provide support to the patient care team as well as directly attending to certain specified needs of the patient.

Responsibilities

  • Provides constant observation and monitoring of assigned patient(s) to ensure safety and Prevention of injury
  • Receives report from exiting patient care sitter, LPN or RN including any patient specific behaviors to monitor.
  • Courteously greets patients, identifies self, and explains purpose.
  • Ensures patient’s safety, regardless of the unit assigned to for the shift.
  • Observes and reports to LPN or RN all patient concerns and changes in condition.
  • Promptly calls for assistance as needed.
  • Provides care within scope of practice.
  • Assists patient to restroom. Walks patient in corridor as allowed/ordered.
  • Does not leave patient unless relieved by another staff member.
  • Utilizing the AIDET model, conducts an appropriate introduction, discusses with patient ADL's and plan for the shift and compliments at least one member of the patient care team each shift. Purpose directly relates to patient satisfaction scores and performance evaluation.
  • Notifies LPN/RN if tubes, IV, catheter, etc. becomes disconnected.
  • Reports defective equipment to LPN, RN or charge nurse.
  • Accompanies patient off the department for all tests and procedures or transfer to another department.
  • Transports lab specimens, patient equipment and patient belongings as required with the patient.
  • Document patient care within the Electronic Health Record (EHR) system as needed.
  • Keeps patient room neat and orderly.
  • Ensures alarms are audible to alert staff, each shift.
  • Evaluates patient’s environment for possible hazards and removes those hazards.
  • Acts rapidly and calmly during emergencies.
  • Washes hands before and after patient contact.
  • Follows isolation procedures according to the Infection Control Policy.
  • Demonstrates understanding of the unit of department technology, i.e. call light system, bed controls and TV controls.
