We are seeking a Patient Care Representative I to serve as the initial point of contact for Capital Health Plan (CHP) members seeking medical care. This role plays a key part in supporting patient access, front office operations, and clinical workflow by assisting with appointment scheduling, registration, documentation, and communication. The Patient Care Representative I answers incoming calls, enters information into the Electronic Health Record (EHR), verifies membership and coverage, and supports patient check-in and checkout processes. This position also assists with collecting patient cost shares, coordinating follow up care, and ensuring documentation is complete and accurate to support quality standards and patient safety.
