Requirements

  • Education Post-Baccalaureate Degree of Pharmacy.
  • Age at least 18 years of age with no current drug or alcohol addiction/convictions of any drug related misdemeanor or felony.
  • Two years of health care pharmacy experience required
  • Current active Pharmacy Licensure (PH) for work state required.
  • Current American Heart Association Basic or American Red Cross Life Support (BLS) strongly preferred at time of hire, required within 3 months of hire.
  • Maintain competency in unique aspects of drug therapy for patients, including factors related to pediatrics, adults and geriatrics.
  • Monitors drug therapy to evaluate appropriateness of use, drug, dosage form, regimen, route, therapeutic duplication and drug interactions.
  • Assessing patient demographics, reviewing patient medication histories; reviewing patient medication profiles; establishing desired outcomes; developing therapeutic care plans, determine alternatives for achieving outcomes; educating patients and/or family on drug therapy and monitoring patient outcomes.
  • Monitoring potential adverse, drug-drug and drug-food interactions.
  • Mastery of use of Cerner pharmacy and hospital information system application.
  • Proficient with all technology associated with the medication management process to include but not limited to Automated Dispensing Cabinets; Anesthesia Carts; 797 IV Admixture requirements.
  • Adhere to policies, procedures, staffing, safety metrics and financial metrics.
  • Maintain compliance with all Iowa and federal regulations regarding medications.
  • Communicate effectively with hospital personnel, patients and the public.
  • Computer skills including Microsoft Outlook and Word.
  • Ability to work alone with minimal supervision.
  • Supervise pharmacy interns and technicians as needed.
  • Ability to prioritize time and tasks according to the needs of patients and the Department of Pharmacy without prompting.

Nice To Haves

  • American Heart Association Advanced Cardiac Life Support (ACLS) preferred.

Responsibilities

  • Clinical Core Competencies Demonstrates competency with the established Clinical Programs approved by the Medical Staff.
  • Transfer of medication issues is communicated between the pharmacy team.
  • ALL issues and pending transactions are communicated to staff for completion.
  • ALL issues and pending transactions communicated to them are completed.
  • Clinical Model adherence (Intervention documentation and observation).
  • Complete all assigned task based on assignment.
  • Drug formulary is preserved (Intervention documentation and non-formulary requests).
  • ALL non-formulary request communicated to prescribing physician with options.
  • Age Specific Criteria incorporated into clinical decision process.
  • Intervention documentation or completion of 1 CE program on age criteria.
  • Screen all patients and adjust therapy options based on age of patient.
  • Renal Dosage Adjustment (Adjustments noted within PharmNet).
  • Adjust all approved therapies on patients whose renal function dictates.
  • Pharmacokinetics Management (PK sheets and intervention documentation).
  • PK sheet correctly completed and entered into PK program.
  • Anticoagulation program (Intervention documentation).
  • Complete monitoring and assessment of all anticoagulation patients as outlined.
  • Operational Core Competencies Effectively manages assigned areas based on established policies and procedures.
  • Oversee pharmacy technicians & interns to achieve flow in the medication process.
  • Prioritize and maintain effective flow of the medication process.
  • Sterile Compounding Management Pass Low Risk Level Competency and manage flow of products.
  • Pass Chemotherapy Management Competency and manage flow of products.
  • Central Pharmacy Management Order verification complete and prompt based on assistance required.
  • Manage the flow of products with no backlog or delays.
  • Teamwork exhibited by actions Expectation: completion of orders to point of delivery (visual inspections while staffing)
  • Safety Competencies Critically manages assigned areas based on established policies and procedures.
  • Hazardous Drugs Demonstrates ability to safely oversee the management of Hazardous Drugs.
  • Central Pharmacy Actions represent the best interest of the patient (observational).
  • Follows all Safety Alerts: Code Red; Code Triage; Code Adam; other codes.
