About The Position

At Atlas Medical, we're growing-and we're looking for a people-first, relationship-driven professional to join our team as a Community Liaison / Patient Care Navigator (PCN). You'll play a key role in expanding access to our full continuum of care with Aegis Healthcare, including Mobile Primary Care, Home Health, Palliative, Hospice, Therapy, DME, and Medical Transport. Requirements

Relationship-builder with 2+ years in marketing, outreach, or referral development
Passionate about healthcare and making a difference
Self-starter who thrives in a fast-paced, people-focused role
Strong communicator (in person, over the phone, and in writing)
Organized, adaptable, and goal-driven
Valid driver's license + reliable transportation
Auto insurance & registration
Ability to pass a Background and Drug Screen

Nice To Haves

Familiarity with the local senior care or healthcare space is a plus

Responsibilities

Grow and manage referral partnerships across your territory
Connect with patients, families, and providers to guide care solutions
Represent Atlas at community events, presentations, and networking opportunities
Identify new growth opportunities and drive strategic outreach
Collaborate with internal teams to ensure seamless patient experiences
