About The Position

The first hello that makes healing feel possible. The Patient Care Navigator is the first, trusted point of contact for clients and families entering YSS Recovery services, creating a safe, welcoming, and compassionate environment from the very first interaction. This role supports clients with empathy, respect, and understanding, guiding them through the admissions and scheduling process while honoring their experiences and choices. Navigators provide clear, patient information about services, connect clients to the right supports, and help them feel seen, heard, and empowered throughout their journey. Under the supervision of the Clinic Operations Director, the Patient Care Navigator manages front desk operations, provides phone and administrative support, prepares client paperwork, and coordinates scheduling for counseling and psychiatry programs. This role provides direct and indirect support to clinical, nursing, residential, and shelter teams, advocating for clients and families as they engage with counseling and medication services. In addition to high-touch client care, the position performs essential administrative, scheduling, and operational functions that keep the Welcome Team running smoothly, with ancillary duties assigned by leadership as needed.

Requirements

  • High school diploma or equivalent.
  • Required Travel is required for this position.
  • For YSS insurance purposes, you must: be 21 years of age, have a current, valid, state-issued, driver’s license, and have a clean driving record.

Responsibilities

  • Welcome clients and families as the first point of contact, creating a safe, respectful, and trauma-informed environment
  • Guide intake, admissions, and scheduling with clarity, empathy, and efficiency
  • Provide accurate information about services and next steps
  • Coordinate scheduling for counseling, psychiatry, and related services
  • Prepare and maintain client documentation in compliance with confidentiality standards
  • Manage front desk operations, including phones, inquiries, and client flow
  • Collaborate with clinical, nursing, residential, and shelter teams to support coordinated care
  • Advocate for clients by helping navigate services and reduce barriers
  • Maintain organized records and scheduling systems
  • Support daily operations and administrative tasks for the Welcome Team
