About The Position

As part of our growing team, you’ll work alongside experienced healthcare professionals while helping coordinate care that makes a meaningful impact in patients’ lives. As a Patient Care Navigator, you will play a critical role in coordinating services and supporting providers after patient visits. Your work helps ensure patients receive the services, medications, and follow-up care they need to stay healthy at home.

Requirements

  • Minimum one year of medical office experience required
  • Experience with front office operations (check-in/check-out)
  • Prior authorization experience (Availity, United Healthcare portal, CoverMyMeds preferred)
  • Ability to work in a high-call, fast-paced environment
  • Excellent customer service and communication skills
  • Proficiency with Microsoft Office

Nice To Haves

  • eClinicalWorks experience preferred
  • Associate’s degree preferred
  • Medical Assistant certification preferred
  • Strong organizational and multitasking abilities

Responsibilities

  • Process provider orders and follow-up tasks after patient visits
  • Coordinate services such as home health, laboratory testing, DME, and radiology
  • Complete prior authorizations for medications and services
  • Process medication refills and communicate updates with providers
  • Collaborate with the healthcare team to ensure continuity of care
  • Maintain compliance with medical record documentation standards
  • Communicate with patients, pharmacies, and care partners to coordinate services
  • Support updates related to home health and other care services
