About The Position

We are seeking an experienced and professional Patient Care Navigator to become our team. You will guide patients through the health care system by assisting with access issues, developing relationships with service providers, and tracking interventions and outcomes. Serve patients and caregivers to provide resources and assistance clinical and supportive care services.

Requirements

• Associates Degree in health care field; Bachelor's degree preferred.
• Minimum 1 year clinical specialty area experience required.
• Current Florida State Registered Nurse license.
• Current BLS (Basic Life Support).
• Critical thinking, service excellence and good interpersonal communication skills.
• Ability to read/comprehend written instructions.
• Ability to follow verbal instructions.
• PC skills.

Responsibilities

• Serve as a single point of contact for referring physicians, patients and caregivers to ensure access to clinical and support care services.
• Assist patients in understanding their diagnosis, treatment options, and the resources available.
• Assist patients and families in establishing a support network and addressing physical, social, and emotional needs.
• Communicates effectively and works with physicians and/or other members of the interdisciplinary team in meeting patient needs.
