About The Position

Summary of Performance Expectations. The Patient Care Manager (PCM) coordinates patient care working synergistically with the inter-professional clinical care team, the patient and family to achieve quality outcomes. Provides evidence based culturally sensitive care, monitors patient and healthcare outcomes and applies best practice nursing strategies. Assists in mentoring direct care staff related to clinical performance and measured outcomes. Participates in resolving patient issues or complaints. Leads and coordinates discharge planning with the inter-professional team. Develops and revises policies appropriate to clinical practice and patient population.

Requirements

  • Graduate of accredited BSN nursing program.
  • Current NC RN license or licensure from Nursing Compact State.
  • BCLS certification. If the BCLS certification is not from the American Heart Association (AHA), an AHA certification is required within three months of employment date.
  • Effective May 1, 2016 newly employed and current employees promoted into the following positions are required to possess a MSN degree or a Master’s degree in a related field.

  • Clinical Development Coordinator
  • Nurse Director
  • Patient Care Manager
  • Professional Development Coordinator
  • Resource Coordinator/Professional Development Coordinator
  • The MSN degree or Master’s degree in a related field must be completed within five years from the date of employment or placement into the position.
  • Five years nursing experience.
  • Served in the capacity of one or more of the following: charge nurse, preceptor, team lead, successfully led projects, or BCLS, ACLS, PALS instructor.
  • Effective 11/01/16, newly employed and current employees transferring into this role are required to possess the five or more years of nursing experience along with experience as charge nurse, preceptor, team lead, successfully leading projects, or BCLS, ACLS, PALS instructor.
  • Given training and on-the-job experience, incumbent should be proficient in the basic aspects of the positions within three months of employment date.

Nice To Haves

  • Obtains national nursing certification in specialty area of practice.
  • Masters in Clinical Nurse Leader; Clinical Nurse Leader certification.
  • Experience in coordination of clinical care in the acute care setting.
