The Home Health Patient Care Manager is responsible for the supervision and coordination of clinical services, providing and directing nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. You will provide clinical services within the scope of practice defined by state laws governing nursing, in accordance with the plan of care, and in coordination with other members of the healthcare team. Your responsibilities will include receiving referrals, ensuring appropriate clinician assignments, evaluating patient orders, and plotting start of care visits. You will also coordinate the determination of patient home health benefits, medical necessity, and ongoing insurance approvals. It is essential to ensure that patient needs are continually assessed and that care rendered is individualized, appropriate, reasonable, meets home health eligibility criteria, and is in accordance with physician orders. Daily reviews of assessments and plans of care will be part of your workflow, and you will consult clinicians with recommendations as appropriate.
