Requirements

• Must have RN license.
• One year prior professional nursing experience in California.
• If acting as a nurse supervisor, requires 2 years experience in the last 5 years in a home health agency, primary care clinic, or health facility.

Responsibilities

• Supervise and coordinate clinical services and provide and direct provisions of nursing care to patients in their homes as prescribed by the physician.
• Ensure compliance with applicable laws, regulations, and agency policies.
• Provide clinical services within the scope of practice as defined by state laws governing the practice of nursing.
• Receive referrals and ensure appropriate clinician assignments.
• Evaluate patient orders and plot start of care visits.
• Coordinate determination of patient home health benefits, medical necessity, and ongoing insurance approvals.
• Ensure patient needs are continually assessed and care rendered is individualized to patient needs.
• Review assessments and plans of care daily and consult clinicians with recommendations as appropriate.
