About The Position

Heritage Hall is now recruiting for a Patient Care Liaison. The Patient Care Liaison supports residents, families, and interdisciplinary team members by coordinating meetings, gathering information, and ensuring timely completion of required documentation. The role assists in promoting resident-centered care, supporting communication among teams, and providing limited direct resident assistance as needed. Strong organizational, communication, and customer service skills are essential to success. Requirements

  • High school diploma required; Bachelor's degree preferred
  • Customer service or healthcare experience preferred
  • Knowledge of medical terminology preferred
  • Basic understanding of long-term care regulations preferred
  • Ability to multitask and prioritize in a fast-paced environment
  • Strong organizational and time-management skills
  • Ability to maintain confidentiality and comply with HIPAA regulations
  • Ability to work collaboratively with interdisciplinary teams
  • Strong oral and written communication skills
  • Computer knowledge and data entry skills
  • Proficiency in Microsoft Word and Excel
  • Professional, polite, and positive interpersonal skills when interacting with residents, families, and team members
  • Knowledge of medical terminology preferred

Nice To Haves

Responsibilities

  • Collaborate with team members to support resident's preferences and individualized treatment plans
  • Schedule and coordinate a variety of supports, including: Care-plan meetings, Resident transports, Equipment needs
  • Assist team members to ensure Care-plan meetings are organized and productive
  • Gather information from residents and families efficiently to meet internal and regulatory timelines
  • Complete required documentation accurately in the database within designated timeframes
  • Maintain a professional and courteous environment for residents, families, visitors, and staff
  • Schedule resident transports
  • Respond to calls in a timely manner
