About The Position

Coordinate and manage care for patients by assisting patients in navigating their care across the continuum to include making annual wellness visit (AWV) and transition of care outreach phone calls. Identify patients who are eligible for a Medicare Annual Wellness Visit and educate the patients on the importance of these visits. Identify at-risk patient populations who have been admitted to the hospital or other in-patient facility, or had a recent ER visit, and communicate with those patients in order to answer any questions and schedule a follow up visit within 7 days of discharge, educating them on the importance of following up with their primary care provider (PCP). Must possess excellent communication and time management skills.

Requirements

  • High School Diploma or GED required.
  • Graduate of a Registered Nursing program, Practical Nursing program, or CAAHEP or ABHES accredited medical assisting program preferred.
  • A current CMA or RMA credential through recognized associations.
  • Current valid NC LPN License.
  • 2 years experience as an LPN or medical assistant preferred.
  • BLS Certification required.
