Access education and advancement opportunities within Stanford’s care delivery team as you gain clinical expertise. • Direct impact in kidney, liver, and lung transplant care: Play a crucial role in delivering smooth, patient-centered outpatient transplant experiences from intake to ongoing follow-up, making a real difference in patients’ health journeys. • Broad clinical exposure: Develop hands-on skills in vital signs, rooming, medication reconciliation, vaccine administration, point-of-care testing, and assist with procedures including stent removal. • Patient-centered culture: Work within Stanford’s C-I-CARE framework to Know Me, Show Me the Way, and Coordinate for Me, delivering compassionate, coordinated care for patients and families. • Collaborate across a diverse multidisciplinary team of physicians, nurses, pharmacists, social workers, dietitians, and support staff. Work Schedule: M-F starting at 7:15am or 7:30am This is a Stanford Health Care job. A Brief Overview As a foundational level, the PCC II, is expected to learn and apply job skills, policies, and procedures to complete moderate assignments and tasks. They should exercise judgment within defined guidelines to determine appropriate actions.
