And that's where you come in. In this role, you'll support a clinical team that's helping patients in new and better ways while giving them a generous helping of compassion and respect. The Patient Care Coordinator (Referrals) is responsible for the completion of set processes and protocols. Works cooperatively with all members of the care team to support the vision and mission of the organization, deliver excellent customer service, and adhere to Lean processes. Supports the teams in meeting financial, clinical, and service goals.

Responsibilities

  • Exhibits empathy, courtesy, competence, efficiency, and care
  • Schedules appointments and procedures, following standard guidelines
  • Uses multiple computer and phone systems to fulfill patient needs over phone
  • Verifies patient information, while documenting in their Electronic Health Record (EPIC)
  • Advises patients of their responsibilities regarding insurance and referral information
  • Greet customers into the practice and provide instruction and/or direction as necessary on the phone
  • Schedule and coordinate all patient appointments as directed
  • Prepare all documentation and records with accuracy as directed or necessary for the patient's care
  • Meet or exceed the performance measures established by the Practice Management Department
  • Follow established policy and procedures related to the position
  • Participate and contribute in a team-based environment
  • Maintain a team supportive attendance record
  • Understand and deliver quality customer service that is consistent with the Polyclinic's efforts in becoming a High-Performance Organization
  • Follow OSHA/WISHA (including usage of PPE) guidelines
  • Follow HIPAA regulations
  • Adapt and perform effectively in a continuously changing environment, and in occasional difficult situations

Requirements

  • High School Diploma/GED (or higher)
  • 1+ years of receptionist experience in a medical office
  • 1+ years of Microsoft Office experience

Nice-to-haves

  • EMR/EPIC experience
  • UCB/ACD Desktop phone system experience
  • Basic knowledge of medical terminology
