As a Patient Care Coordinator, you will support doctors and patients through a variety of tasks related to patient case management, organization and communication. The goal is to improve the health of low-income pregnant women and as a result, achieve the best possible outcome of every pregnancy by conducting health, nutrition, and psychosocial screenings and providing supplemental education. The ideal candidate is community-oriented, and has a desire to excel and proactively participate in patient care by working directly with the patient to create an individualized care plan and provide referrals to community resources based on patient needs; such as WIC, dental, or counseling services. The target is to complete all tasks accurately, respectfully, and in a timely manner. Comprehensive one-on-one training provided. Second language preferred, but not required. Previous experience in medical office or obstetrics preferred, but not required.
