About The Position

CenterWell Home Health, a Humana company, is seeking a Patient Care Coordinator to join their caring community. This role is crucial for developing and maintaining relationships with discharge planning services at hospitals, skilled nursing facilities, and assisted living facilities to provide updates on CenterWell's home health services. The Patient Care Coordinator will primarily conduct facility visits at physicians' requests to assist the program clinical team in determining patient eligibility for home care. The position involves ensuring effective communication and collaboration with program staff and field sales resources, actively participating in weekly business development meetings, and assisting the program in the timely processing of physician orders. Requirements

  • RN/LPN/LVN license
  • Strong knowledge of governmental regulations, Medicare eligibility requirements, comprehensive understanding of potential care plan needs for the patient and coordination of necessary resources
  • Excellent customer service, account development capabilities, organization, time management, problem-solving, communication and selling skills

Nice To Haves

  • Active Nursing License
  • A minimum of three years clinical experience

Responsibilities

  • Develop/maintain contact with key hospital, skilled nursing, assisted living facility discharge planning services and/or management to provide ongoing updates on Company’s services available in a market
  • Primarily conduct facility visits at the physicians' request to assist program clinical team in determining eligibility
  • Ensure effective communication and collaboration with program staff and other field sales resources via weekly meetings
  • Actively participate in weekly program business development meetings, bringing relevant data, reports, as well as information regarding changes within accounts and referral sources
  • Assist program in timely processing of physician orders as directed
